Free HPV vaccination! A national commitment to protecting women's health?

  Xinhua Net reported on October 30, 2025, that starting from November 10, 2025, China officially included the HPV vaccine into the national immunization program. Girls who are 13 years old after November 10, 2011, can receive the bivalent HPV vaccine for free. The headline of the news is “Free HPV Vaccination! A National Commitment to Protect Women’s Health,” and the article loudly promotes this move as entirely aimed at “practically safeguarding women’s health.” But is this really the case?

  The human papillomavirus (HPV) vaccine (English: HPV vaccine, also known as HPV vaccine) is used to prevent infection with certain types of human papillomavirus (HPV), preventing diseases such as cervical cancer caused by persistent high-risk HPV infection. According to estimates, over 95% of cervical cancers are caused by sexually transmitted HPV. Cervical cancer is the fourth most common cancer among women worldwide and also the fourth leading cause of cancer death among women. In 2021, there were approximately 604,000 new cases of cervical cancer globally, resulting in about 342,000 deaths. In recent years, China has seen a rising incidence and younger age of cervical cancer patients. In 2022, there were about 151,000 new cases and approximately 56,000 deaths from cervical cancer in China.

  Among the many factors leading to cervical cancer, besides HPV infection, there are also smoking, immunodeficiency, frequent use of contraceptives, unsafe sexual behaviors, multiple sexual partners, etc. In capitalist societies heavily influenced by pornographic culture, the main perpetrators of “unsafe sexual behavior” are men who seek sexual stimulation through watching pornography, masturbation, or even prostitution. Of course, it is not excluded that some women in these societies, fallen into depravity, engage in prostitution and also partake in “unsafe sexual behaviors,” but as Engels wrote in “The Origin of the Family, Private Property and the State”: “Prostitution merely makes unfortunate women victims of degradation, and even they are far from the extent of degeneration usually imagined. On the contrary, it corrupts the character of all men.” Although medically, HPV infection is considered more critical than other factors, it is clear that the widespread transmission of the virus is also related to “unsafe sexual behaviors.” Therefore, the increase in women diagnosed with cervical cancer in recent years can be attributed to the rampant porn culture in capitalist societies. The morally corrupt men are the real culprits for infecting their partners with HPV and causing cervical cancer, yet these harms are often borne entirely by women.

  “Women can prevent about 90% of HPV infections if vaccinated before their first sexual experience.” China has long promoted the necessity of HPV vaccination in this way. Incorporating the HPV vaccine into the national immunization plan and providing free vaccination for eligible women seems to be a just move to protect women’s health. Most major news media reports on this matter show public support and recognition, with some lamenting that they missed the government’s welfare policy and that paying for the vaccine earlier was a loss. However, some comments question: “Why not let men get vaccinated too?” Data shows: “Men are the main spreaders of HPV, with a higher infection rate than women. A model-based study of the U.S. population estimates that the lifetime probability of HPV infection for men is 91.3%, and for women, 84.6%.” Harald zur Hausen, the father of the HPV vaccine, openly admitted, “Men do not get cervical cancer, but they are the main transmitters of HPV. Vaccinating men can protect their partners and also prevent other cancers caused by HPV, thus protecting themselves.” In 2019, the UK government announced plans to vaccinate boys. The CDC in the United States recommended in 2019 that all children aged 11-12 receive the HPV vaccine, which can prevent over 90% of HPV-related cancers. Countries like the UK and the US have long recommended vaccination for both genders. However, China not only only introduced relevant policies on October 30, 2025, but also only offers free vaccination to 13-year-old girls born after November 10, 2011. It has always promoted only female vaccination to prevent cervical cancer, never highlighting that men are the main carriers and transmitters of HPV, nor allowing men to be vaccinated equally to reduce transmission risks. This leads many uninformed people to mistakenly believe that HPV infection is solely a female issue, ignoring men’s role in spreading the virus, and preventing men from taking measures to avoid infection, which is a covert victim-blaming logic. Such an extreme reactionary Confucian society not only refuses to recognize that only vaccinating women is discriminatory and victim-blaming but also fosters a public opinion that this policy is “special care” for women. Just look at the comments sections of major media outlets for proof.

  As early as 2010, the Japanese government included the HPV vaccine in the national immunization program, allowing women over 12 to receive it for free. With the widespread vaccination, some regions exposed adverse reactions among women vaccinated with HPV, with media statistics showing that among 100,000 people, about 15.6 experienced symptoms such as widespread pain and difficulty standing. As these victims were exposed, many people protested and refused further vaccination. People in countries that previously promoted free HPV vaccination have gradually realized the true situation of the vaccine, becoming more cautious and less blindly trusting official and bourgeois expert claims. This is reminiscent of the COVID-19 pandemic years ago, when many people experienced negative effects after receiving China’s substandard vaccines, realizing the reactionary nature of the government. The exposure and imprisonment of unscrupulous vaccine sellers could not undo the damage done to people’s health. The quality and efficacy of HPV vaccines remain uncertain. Even if we temporarily ignore whether they can truly protect women, and accept that they are some kind of “improvement” by the government, it is clear that the government only seeks to create a righteous stance of “full protection of women’s interests” to deceive the masses, especially women, and to cover up the societal reality of patriarchy oppressing and discriminating against women everywhere. Which society, like China, would beautify discriminatory and oppressive acts as “special care” for women? Which society, like China, would tolerate and openly promote discrimination against women while claiming to support gender equality, even with the national leader attending the so-called Global Women’s Summit and delivering keynote speeches? Only in the most reactionary and hypocritical Confucian society can such a hypocritical and reactionary scene unfold.

After conducting some investigations based on this news, I can provide some additional information. Cervical cancer is caused by high-risk HPV infection in 99% of cases, which means that HPV infection leads to cervical cancer. If viral infections can be thoroughly prevented, this disease that threatens women’s lives can almost be eradicated (cervical cancer is the second most common cancer among women, after breast cancer). There are three transmission routes for HPV: sexual transmission, mother-to-child transmission, and skin-mucous membrane contact, with sexual transmission accounting for 70%–80% (1). Anyone with a basic understanding of disease prevention knows that to eliminate an infectious disease, three things are needed: eliminate the source of infection, block transmission routes, and treat infected individuals. From these three aspects, it is not difficult to understand how to completely stop the spread of HPV. Since HPV is not transmitted by livestock or parasites, eliminating HPV does not require large-scale extermination campaigns like those for plague or malaria. Instead, efforts should focus on treating infected individuals and blocking transmission routes. Currently, there are no specific drugs for HPV, so the most effective methods are already clear: preventing sexual transmission and vaccination.

The Chinese government repeatedly claims to be protecting women’s health, but why does it only promote vaccination and not block transmission routes? It is evident that the government understands the importance of blocking transmission in eradicating infectious diseases, as evidenced by the recent pandemic measures—mandatory quarantine, mask mandates, and the sealing of residential buildings with steel bars—just to “prevent infection.” This demonstrates how much manpower and resources have been投入 for “epidemic prevention.” However, when it comes to eradicating HPV, they are stingy and hesitant, only vaccinating girls from the age of 13 (since birth in 2011). The government has not provided a clear reason, only claiming that this is the “optimal solution” considering “protection effects, cost-effectiveness, and feasibility” (2). But is this truly “protection”? What exactly does “cost-effectiveness” refer to? With careful thought, the truth becomes evident.

As mentioned in the article, men can also be infected with HPV, and their infection rate is even higher than that of women. Men are the main transmitters of HPV, and after infection, they can develop diseases like condyloma acuminatum and anal cancer. If we truly consider “cost-effectiveness,” wouldn’t it be better to vaccinate boys for free? The government claims this move is to curb the “youthful” and “widespread” nature of cervical cancer. Regarding the latter, we have already raised questions. As for the former, a survey in Beijing shows that the youngest woman diagnosed with cervical cancer was only 17 years old (3). Assuming these women are infected with high-risk HPV, it takes 1–3 years to develop into cancer. This means that women diagnosed at 17 likely had sexual activity by the age of 15 or 16 at the latest. Some might argue: what if such cases were infected through non-sexual transmission routes? However, HPV testing is not part of routine examinations for young women, and in a country like China, where Confucian values prevail, men can cheat or visit prostitutes, but women are expected to adhere to “feminine virtues.” Women often worry about social pressure when undergoing gynecological exams; they fear suspicion and blame for being “immoral,” and face social rejection. They usually do not spend money on specialized hospital checks. In rural areas where Confucianism is even more prevalent, this is even more common. Therefore, it is very realistic that young women are infected with HPV through sexual activity, and the number is likely quite large, possibly even younger than estimated by the government.

To prevent the “youthful” trend of HPV infection, even if we adopt a piecemeal approach like treating symptoms as they appear, the most obvious measure is to raise the legal age of sexual consent. However, the government has not done so. While claiming to prevent the “widespread” and “youthful” spread of cervical cancer, they do not block HPV transmission routes and only vaccinate women, specifically young girls. If we think further, considering China’s recent plummeting birthrate and the rise of various mass movements, the answer becomes clearer. The government’s rhetoric about “protecting women’s health” is actually aimed at maintaining their reproductive capacity, because if cervical cancer occurs, it will severely damage women’s reproductive systems. The so-called goal of preventing “youthful” and “widespread” cervical cancer is essentially to prevent the decline of birthrates.

As a fascist regime, the government seeks to maintain its rule by buying support and expanding its social base. The legal age of sexual consent remains at 14, one of the lowest in the world—only slightly higher than Japan, which has a developed sex industry and raised its age to 16 after social protests. The Chinese government stubbornly maintains this law. Lenin once exposed that constitutionalists talk about distributing land to peasants but never mention from whom they will take it, effectively promoting the idea of class extinction and nationalism, knowing well that land should be transferred from landlords and rich farmers. If they actually did so, it would undermine the Tsarist regime’s foundation. Similarly, the current regime’s move to keep the age of consent low is to avoid losing support from the right-wing petty bourgeois male faction, which is crucial for maintaining their rule. Therefore, they adopt the most “cost-effective” approach.

Thus, the so-called “cost-effective” method to curb cervical cancer is actually the most “cost-effective” method for ruling and exploiting the people—the so-called “protection” of women’s health is in fact protecting the reactionary regime’s “health.” This approach delays the decline of birthrates, preserves the interests of the right-wing petty bourgeois male faction, and promotes nationalist ideology, achieving multiple goals with one move. No wonder the regime promotes it so vigorously.

They only vaccinate women but do not mention that men are the main transmitters of HPV. The reasons given for sexual transmission include “early sexual activity” and “multiple partners,” implying that women are solely responsible. This is the most despicable and shameless insult to women who are oppressed by gender inequality.

Having discussed this, our understanding of the essence of cervical cancer deepens. Its persistence today is entirely due to the decayed capitalist system and reactionary patriarchy. Like other public health hazards under capitalism, instead of calling it “cervical cancer,” it could be called “capitalist cancer” or “patriarchal cancer.” Historically, this is true. The increasing youthfulness of cervical cancer correlates with China’s capitalist restoration and imperialist shift: in the early 1990s, the high-incidence age was 61–65; by 2000–2008, it shifted to 31–55, accounting for about 78% of cases. A 2021 epidemiological study shows that the main affected group nationwide and in urban and rural areas is women aged 25–45 (4). Only by overthrowing the current oppressive regime, establishing a socialist government where the people are masters, can cervical cancer be eradicated once and for all, and this plague be sent away.

Appendix: During Mao Zedong’s leadership of the Great Leap Forward in socialist China, efforts extended beyond industry and agriculture into medicine and health. The masses, full of enthusiasm for building socialism, made a bold vow to “defeat cancer within five years” and “make cancer bow.” To care for the women who make up half the sky, cervical cancer was a primary target during this medical and health leap. The people united in a “Great Leap Forward” speed, conducting testing and surveys on 100 million people nationwide. Originally planned to be completed in three months, the survey was finished in just three weeks, including investigations into cervical cancer. Over 1 million women over 25 in 20 cities underwent screening, with participation rates between 39% and 69%. During screening, cadres also cared for their families. By 1976, China had identified HPV as the culprit of cervical cancer. Eradication of this cancer was within reach, but after the capitalist restoration, Chinese women returned to a state of oppression (5).

Administering HPV vaccines to young girls is completely to facilitate and embolden rapists to sexually assault young girls. In fact, these disgusting reactionary beasts have never cared about the immense harm caused to women. These sources of infection should have been eradicated long ago.
